Children / Kids Foot Pain

Foot pain in children is common and often related to growth, activity levels, and developing biomechanics. While many cases are temporary, ongoing pain should be assessed to prevent long-term issues.

Common Conditions

  • Sever’s disease (heel pain related to growth)
  • Flat feet (paediatric pes planus)
  • Overuse injuries from sport
  • Tendon or muscle strain
  • Growing pains affecting the lower limbs

Symptoms

  • Heel pain or arch pain
  • Limping or altered walking pattern
  • Pain during or after activity
  • Stiffness, especially in the morning or after rest
  • Reduced participation in sport or play

Diagnosis

We perform a comprehensive paediatric assessment tailored to the child’s age and development. This may include:

  • Review of growth history and activity levels
  • Assessment of foot posture and alignment
  • Joint mobility and range of motion testing
  • Muscle strength and flexibility (particularly calf muscles)
  • Gait analysis (walking and running pattern)

Where clinically indicated, imaging such as X-ray or ultrasound may be recommended to assess structural concerns or rule out more serious conditions.

Causes & Risk Factors

  • Rapid growth phases (growth plate stress)
  • High levels of sport or physical activity
  • Flat feet or developing foot posture
  • Tight calf muscles (common in growing children)
  • Poor or unsupportive footwear
  • Sudden increases in activity or training load
  • Previous injury

Treatment

Management is tailored to the child and focuses on reducing pain while supporting healthy development. This may include:

  • Education for parents and children
  • Activity modification and load management
  • Footwear advice appropriate for age and activity
  • Stretching and strengthening programs
  • Orthotics where clinically indicated
  • Return-to-sport guidance to prevent recurrence
